Child Medical Consent Form

A parent's written permission for another adult to get medical care for their child.

Good for: Parents who leave a child with a grandparent, relative, babysitter, coach, or other adult who might need to get medical care for the child.

Before you sign

  • Give the adult a copy, and keep a copy yourself. Many caregivers also like to carry a copy of the child's insurance card.
  • Hospitals and doctors in some states want this form signed in front of a notary. The state tips on the last step cover your state.
  • This form does not give the adult any other rights over your child.
